Hydrocephalus (Water on the Brain): Symptoms, Causes, Treatment and New Research

Hydrocephalus, often called “water on the brain,” is a serious neurological condition where fluid builds up inside the brain and can cause dangerous pressure. Doctors traditionally believed this happened because the brain couldn’t properly absorb cerebrospinal fluid (CSF).

However, new published research challenges that idea. Research suggests that the real problem isn’t fluid buildup itself, but how the brain handles the natural pulsing force created by each heartbeat. According to this new theory, hydrocephalus may occur when the brain can’t absorb or manage that pulsating energy correctly.

This matters because current treatment usually involves surgery to place a shunt that drains fluid from the brain — a solution that often requires multiple follow-up surgeries due to complications. If this new theory is proven correct, it could lead to entirely new ways to treat hydrocephalus that focus on managing these pulse forces instead of just draining fluid. According to the Hydrocephalus Association, more than one million people in the U.S. are living with this condition.

What Causes Hydrocephalus?

Doctors have long believed hydrocephalus is caused by problems with how CSF flows through the brain. This traditional view still explains many cases and helps guide treatment today.

Common causes include:

  • Blockages that prevent normal fluid flow
  • The body not absorbing CSF properly
  • Rare cases of excess fluid production

These issues can result from infections, bleeding in the brain, tumors or injury.

However, the research suggests there may be more to the story. We now believe hydrocephalus may also involve how the brain handles pressure from each heartbeat. Every heartbeat sends a small pulse of energy into the brain’s blood vessels. Normally, the brain absorbs this energy smoothly.

The research suggests:

  • The brain may not absorb pulse energy properly in hydrocephalus
  • This may lead to pressure buildup over time
  • Fluid accumulation does not explain symptoms

This shift in thinking could lead to new types of treatments in the future.

What Are the Symptoms of Hydrocephalus?

Symptoms can vary widely based on age because the skull responds differently to pressure at different stages of life. In babies, the skull is still soft, so it can expand as pressure builds up. In adults, the skull is firm, so pressure rises more quickly inside the head.

Symptoms in Babies and Children

Parents may notice physical and behavioral changes early on. A rapidly growing head is often the first sign in infants.

Common symptoms include:

  • Unusually large or fast-growing head
  • Vomiting or poor feeding
  • Excessive sleepiness

  • Downward gaze (“sunsetting eyes”)
  • Irritability or constant crying

Symptoms in Adults and Older Adults

In adults, symptoms often affect movement and thinking. A common form in older adults is normal pressure hydrocephalus (NPH), which can develop slowly and be mistaken for other conditions.

Common symptoms include:

  • Difficulty walking (shuffling steps)
  • Memory loss or confusion
  • Loss of bladder control
  • Headaches
  • Blurred or double vision

Because these symptoms overlap with other conditions, it is important to seek medical evaluation for an accurate diagnosis.

How Is Hydrocephalus Treated?

Hydrocephalus is usually treated with surgery to reduce pressure on the brain. While there is no cure yet, treatment can help manage symptoms and improve quality of life.

Main treatment options:

1. Shunt System: The most common treatment is a shunt, which drains excess fluid from the brain to another part of the body, usually the abdomen. A valve helps control how much fluid is removed.

2. Endoscopic Third Ventriculostomy (ETV): This procedure creates a new pathway for fluid to flow inside the brain. It may reduce or eliminate the need for a shunt in some patients.

Important things to know about treatment:

  • Surgery is the standard approach
  • Ongoing monitoring is required
  • Treatment manages symptoms but does not cure the condition

What Are the Challenges With Current Treatments?

Although current treatments are effective, they are not perfect. Many patients face ongoing issues related to their treatment over time.

Shunts, in particular, can fail. They may become blocked, break or lead to infection. When this happens, symptoms can return quickly and require urgent care.

Common challenges include:

  • Shunt malfunction or blockage
  • Risk of infection
  • Need for repeat surgeries
  • Lifelong medical follow-up

Even procedures like ETV are not suitable for everyone, and results can vary.
